Job Description

Position: Non-Exempt Hourly

Reports to: CA Lead

Direct Reports: None

Job Description:

The Client Associate (CA) is responsible for monitoring and maintaining the daily activity of clients. This position works closely with the Client Associate Lead to ensure clients are in compliance with all expectations and requirements of their daily routine as clients of Foundation Stone, LLC Family of Programs. Your primary responsibilities will be to assist the staff in the management and execution of the daily schedule and coordination/facilitation of client needs. You will also be maintaining files, facilitating and coordinating off-facility activities such as driving clients to medical and other appointments, making airport runs, etc.

Knowledge, Skills and Abilities:

  • Understands food service handling for meal preparations
  • Knowledgeable of client operations; motivating, developing and directing clients to fulfill daily activities successfully
  • Knowledgeable of client rights, confidentiality and boundaries with clients
  • Able to clearly communicate clients’ needs and risks to medical, nursing, and counseling staff
  • Familiar with self-administration and medication observation guidelines
  • Familiar with Waived Testing in relation to point of care drug testing guidelines
  • Physically able to lift 30 lbs

Responsibilities:

  • Responsible for understanding and following all company policies and procedures,
  • Act as a Liaison between patients and staff and relay pertinent client information, including observations, to appropriate programming staff,
  • Supervise daily patient activity and always act in a manner sensitive to their needs and best interest,
  • Provide thorough, accurate, and timely documentation of any services delivered to clients,
  • Develop therapeutic rapport with the milieu and maintain healthy professional boundaries with clients,
  • Maintain a daily communication log for any issues that need to be addressed,
  • Attend and supervise off-site meetings and outings,
  • Supervise and delegate meal preparation and clean up,
  • Inspect building and property for safety concerns and reports to supervisor,
  • Able to assesses for at-risk factors (suicide and self-harm) and intervenes appropriately in accordance with policy and procedures,
  • Maintain the safety of the clients and the security of the facility,
  • Maintain the cleanliness and functionality of the property and facilities,
  • Respond to client injuries or accidents immediately, notifying proper authorities,
  • Secure medications and observe self-administration of medications in accordance with policy and procedures,
  • Protect client confidentiality in accordance with company policies,
  • Ensure that all clients are properly trained prior to being assigned chores,
  • Monitor checklists for accuracy and effectiveness,
  • Transport clients to appointments, as needed,
  • Practice all vehicle safety measures when transporting clients,
  • All other duties as assigned.

Minimum Requirements:

  • High school diploma or GED equivalent, or 1 year working experience
  • Current CPR Certification or within the first 90 days of employment
  • If in recovery, must have a minimum of one year sobriety, and be actively working a program of recovery.
